Receiving error "An item with the same key has already been added"

I have set up an Upsert operation to push data from a CSV file into Salesforce. When I try to run the package I am getting an error every time

Error message: An item with the same key has already been added

Package Name: "Test"

Run Id: 73214273

I have already tried clearing the metadata cache but it hasn't made any difference. Please help
