Submit form connecting to MySQL

My goal is to be able to create a form on a Shopify page that connects to my MySQL database and in turn re-directs the user to their respective page within the website (i.e. they get re-directed to a collection page on Shopify). Can I accomplish this with the Skyvia Query feature?

Please describe your scenario in more details. Do you want to create a form via Skyvia with the corresponding information from MySQL? Or do you want to create a form in Shopify that would be able to connect to MySQL when user open it?

I would like to create a form/login in Shopify that would connect to MySQL. Shopify's templates don't accept PHP so that's where my need has come in, other than this it seems that I would have to create a "private app" and run the code from the page on my own server.  

An example case would be, say the login just has one field for a PIN with a submit button.  The user enters the pin 'AFM313' and the page connects to a MySQL I have set up and re-directs them to a respective link within the website. (The link would just be means to guide them to a particular collection I have set up for them).  


The html of the form would be something as simple as below, I just need to connect it to a database I have set up.

Thank you for the reply and additional information. Unfortunately, this scenario cannot be implemented via Skyvia.
